How Our SNP Model of Care Supports You
2025 Performance Highlights
At Perennial Advantage, our Medicare Advantage Special Needs Plan (SNP) is built to support members with ongoing or complex health needs. We do this through our Model of Care. This approach puts you at the center of your care and helps make services easier to understand and use.
What Is a Model of Care?
Our Model of Care helps make sure you get the care you need, when you need it. It focuses on:
- Working together with your doctors, hospitals, and pharmacies
- Creating a care plan based on your health needs and goals
- Checking in with you after hospital stays or major health changes
- Using a care team to help manage your care
This team based approach helps you get the right care at the right time.
How Did the Plan Perform?
In 2025, the plan showed strong overall performance in many key areas. We are proud of the progress made to support our members.
Performed Well
- Medication Review | Members received medication reviews to make sure their medicines were safe, worked well together, and supported their health.
- Adult Preventive Care Visits | Providers helped make sure members had visits regularly. These visits help find health concerns early and support long-term health and wellness.
- Understanding Daily Living Needs | Members completed assessments to help the care team understand how they manage daily activities. This information helps create a care plan that supports independence and quality of life.
Opportunity Identified
- Interdisciplinary Care Team (ICT) Meetings | ICT meetings help bring members and their care team together to support their health. Although many members had a completed meeting, results were below our goal. We are working to increase these meetings to strengthen teamwork and better support member care.
