FAQs

How do I find out if my primary care provider (PCP) or specialists are in-network?

You can access a list of providers in-network with Perennial Advantage by visiting: https://perennialadvantage.com/find-providers/#/

How do I access my hearing benefits?

Perennial Advantage Freedom’s hearing benefit is administered by Nations Benefits. You can sign in to Nations Benefits Member Support by clicking this link or calling 833-689-1751 (TTY: 771).

Step 1: Find a provider. Log in to your Nations Benefits account to access a list of in-network providers.
Step 2: Make an appointment with your preferred in-network provider.
Step 3: Attend your appointment! Your provider will work with you to understand how to access your hearing aid benefits.

How do I access my vision benefits?

If you have questions about finding a vision provider, call Perennial Advantage’s Member Services at 1-844-751-6003 (TTY: 771).

Step 1: Find a participating provider by visiting Perennial Advantage’s provider directory and search for “optometry.” This will locate eye health providers in your area that work within Perennial Advantage’s network.
Step 2: Make an appointment with your preferred in-network provider.
Step 3: At your appointment, ask your provider to bill Perennial Advantage.

If you do receive a bill for services, you must complete this Medical Reimbursement (DMR) form.  Mail your completed claim to:

How do I access my dental benefits?
Perennial Advantage Freedom’s dental benefits are provided through Liberty Dental.
To access information about your dental benefits, visit: https://client.libertydentalplan.com/perennialadvantage
You can access your claims and other member information by signing in to Liberty Dental’s member portal. If you have questions about your benefits or need assistance scheduling an appointment, call Liberty Dental’s member service representatives at 866-544-1942, TTY: 877-855-8039.
What is the Healthy Living Flex Card?

You have access to a Healthy Living Flex Card, provided by the Helper Bees, to use for over-the-counter items, over-the-counter hearing aids, and groceries.** Every three months, your card will be reloaded with your quarterly allowance to spend on approved items at approved retail locations.
To access your account, check your balance, and more, you can log in to your Helper Bees account at this link. You can also call the Helper Bees directly at 833-284-2435.
**This benefit is only available to those with certain chronic conditions.

I’d like to schedule a companion/in-home support visit or transportation included with my plan. How do I do that?

You can schedule a companion/in-home support visit and transportation through the Helper Bees by logging in to your Helper Bees Member Portal or by calling the Helper Bees at 833-284-2435.
